CANCELLATION POLICY

Our new-patient office consultation fee is $75, due upon scheduling your new-patient visit. (Existing patients may have that fee waived as thanks for their demonstrated loyalty to our practice.) The new-patient consultation fee may be applied to any scheduled procedure. This fee is nonrefundable if the consultation visit is cancelled.

Payment in full is expected when you schedule a procedure, as our team begins staffing and scheduling your procedure as soon as you select a date. For this reason, scheduled surgery and treatment packages are nonrefundable.

INSURANCE

Dr. Samuels does not accept health insurance for treatments or services. However, the office can assist you with filing a claim for pre-authorization to cover selected procedures, such as breast reductions or upper lid blepharoplasty, to qualified patients. Since Dr. Samuels offers cash discounts to qualified patient procedures covered by insurance, our office does many breast reductions and upper lid blepharoplasties at patient request; in most cases, those patients usually face only a slightly higher out-of-network cost.

Financing

Many patients choose procedure financing in order to schedule treatments at attractive payment terms. Dr. Samuels is partnered with industry-leading healthcare finance companies: Care Credit, Patient Fi and Alphaeon. You can easily apply for procedure financing online during your office visit or after you receive a quote from us. Our New Patient Coordinators are happy to assist you with financing options at any time.
